Great Life Care operates through a hub-and-spoke model. A group of experienced clinicians acts as our Expert Advisory Group, supporting our care teams with training, regular clinical supervision and governance oversight. This helps us monitor quality, strengthen competence and respond to each client’s changing needs.

Consultant in Rehabilitation Medicine

Provides clinical leadership for complex rehabilitation packages, including oversight for spasticity management, medication reviews and onward referrals where needed.

Consultant Physiotherapist

Supports neurorehabilitation, 24-hour posture management, spasticity management and therapy-led guidance for people with neurological injuries and disabilities.

Respiratory & Neuro Disability Nurse

Advises on complex neurodisability, tracheostomy care, ventilator dependency, PEG/RIG support, catheter care, epilepsy, pressure ulcer prevention and behaviour management.

Consultant Occupational Therapist

Supports functional rehabilitation, cognitive assessment, complex behaviour management, home adaptations, wheelchair assessment and specialist equipment needs.

Disorders of Consciousness Specialist

Provides specialist experience for people with severe brain injuries and prolonged disorders of consciousness, supporting careful assessment and long-term management.

Wider MDT Support

Our advisory network includes rehabilitation medicine, physiotherapy, occupational therapy, specialist nursing, speech and language therapy and neuropsychology input.

For people living with brain injuries, spinal cord injuries, cerebral palsy, disorders of consciousness, polytrauma, amputations, complex orthopaedic needs or respiratory dependency, care cannot be fragmented. It needs to be planned, reviewed and joined up.

Our case management approach helps individuals and families navigate specialist services, rehabilitation goals, daily routines, equipment needs such as a homecare hospital bed where appropriate, and the practical details that shape everyday wellbeing.

The outcome is simple: safer care, clearer communication and a plan that supports the person’s clinical needs without losing sight of who they are.

We believe in the power of staying active, so exercise is an important part of our care plans. When our caregivers visit your home, they will encourage you or your loved ones to engage in activities that help improve mobility, strength, and independence. And, just as importantly, we set goals for you to work on between our visits, all under the supervision of our registered manager, who is also a professionally qualified physiotherapist.
